Sure.  Different life experiences, for starters.

We're attached to different sensory systems in separate locations.

Must be a great many things, assuming anything was determined at all.

age.
IDK, good questions.  Perhaps whatever we are -is- transferable...might not even need another human to pull the switcharoo with.

Because your mother and father didn't live a thousand years ago, and won't be alive in a thousand more?

You;re asking about being, so ofc metaphysics will be necessary, but I doubt that you'll end up with a very accurate statements thereof without reference to the world we live in, or the method by which we generate sound propositions. More of an "and" than an "or".
I am the Infantry. I am my country’s strength in war, her deterrent in peace. I am the heart of the fight… wherever, whenever. I carry America’s faith and honor against her enemies. I am the Queen of Battle. I am what my country expects me to be, the best trained Soldier in the world. In the race for victory, I am swift, determined, and courageous, armed with a fierce will to win. Never will I fail my country’s trust. Always I fight on…through the foe, to the objective, to triumph overall. If necessary, I will fight to my death. By my steadfast courage, I have won more than 200 years of freedom. I yield not to weakness, to hunger, to cowardice, to fatigue, to superior odds, For I am mentally tough, physically strong, and morally straight. I forsake not, my country, my mission, my comrades, my sacred duty. I am relentless. I am always there, now and forever. I AM THE INFANTRY! FOLLOW ME!

You're simply rephrasing the idea.

What determines "my parents";  "my sensory system"...etc in relation to yours prior to either of our extant circumstances?

How about you tell us? Because I have a feeling you're building up to it anyway.

No consciousness without the brain, no afterlife, no metaphysical woo involved.

By what environs and contingencies are these "roll of the dice" made....and who or what is rolling them?

Genetics.  Chance.  My Father met my Mother - would have never happened if he had not signed up for a summer apprenticeship in Nashville.  Pure chance.  Human biology.  No "who or what" or any outside influence needed, and none in evidence.

There, you answered your own question.

Well no, not really.  Just yours.

Quip, if you're looking for reasonable potential factors, others have listed some in this thread already (chance, genetics, the obvious difference in personal experience, etc). If you're looking for a single, external, metaphysical answer well.. I dunno.

Missing the point.

What determined this particular gene pool as "yours"?

There is no me and you or us to compare or contrast "prior to our extant circumstances".  The question is nonsensical in context. Rephrase?
