I think Brian's mystery episode might have been The Lights of Zetar. Nobody lost their faces but they became distorted and there were gargling, groaning sounds.
Only two things are infinite, the universe and human stupidity, and I'm not sure about the former.

Actually, you’d expect it to be a goldshirt. Attend:

The Enterprise had 239 redshirts, 25 of whom died on screen. This is a mortality rate of a little more than 10%.  However, out of the 55 goldshirts aboard, 10 died on screen, for a rate of 18%.
