For the last few years I have been suffering from fatigue. I felt exhausted all the time but I put it down to stress and hard work. I wasn't drinking any caffeine. Turned out to be multiple sclerosis and lesions in my corpus collosum. The brain has to find alternate, less efficient routes to compensate which uses up more metabolic energy.
Try healthy eating with plant based foods, cut down on dairy and saturated fats and take Vitamin B supplement, and also regular exercise. Doing that stopped my fatigue. It also helps against many diseases of affluence that we suffer from in the west.

(I can sense damage on my teeth and they start to kill and my gums start to bleed sometimes, straight after I eat ANYTHING like that, that's how bad it's gotten (and I've already had one wisdom tooth removed, and another one that is killing like a bitch every day now, I am sure needs removing too). All the ice cream and orange juice and energy drinks were not worth it
