I grew up in an Eastern Europe, where weed and hashish were very rare. We used to smoke it in glass tubes - cigarette holders - because it's less wasteful than joints and we would mix it with tobacco, mainly because that way it was easier to tightly load the tip of the tube, without bits of weed falling out half-burned, or being sucked in.
In UK, we usually smoke joints and we mix weed with tobacco, especially if it's a strong kind, like skunk. It makes it easier to hold the smoke without coughing, especially if you're a tobacco smoker. Also, if you're smoking slowly a joint keeps burning away, while you're not dragging on it, so if all there is in it is weed - you're wasting a bunch of it.
I think a lot of smoking culture differences stem from the availability of marijuana and I'm pretty sure it's more easily available (and consequently cheaper) in US, especially now with all those "medicinal" weed dispensaries.
