NPD is the creation and maintenance of a false self, usually to brace or buffer ones actual self from the outside world, to the detriment of that actual self and all those around you. It's called narcissistic personality disorder for the inordinate amount of effort spent on that self, not because the person is..as we use it in common parlance, a narcisist. How could they be, they're not admiring themselves (precisely the opposite, they generally loath themselves)....they're attempting to convince -you- to accept and admire the false self. It's associated, with disassociative breaks and disorders (lol), depression...substance abuse...an inability or difficulty to empathize with others, and may in fact be genetic and inheritable (or a propensity for it may be genetic...as it's understood that a person can be conditioned by themselves, others, or events to present this disorder even if there is no family history of it - though it is most commonly thought to have it's onset in early childhood).
Being a narcisist, otoh, is just run of the mill assholery.
(I had a run in with a diagnosis back in my hooah days. Disassociative, NPD, depression, substance abuse. Spare those of us that have felt this the indignity of being associated with trump just because he's a garden variety asshole.)
