This is my opinion:
First of all it needs to be understood that Heaven is not a physical place. It is best described as a "state of being," if you will. Heaven is when our hearts and minds reach a stage that is consumed with pure love and goodness. We are super close to God, which is Himself the source of love and goodness. Since we were made for love and from love, reaching the stage of being consumed by it makes us perfectly happy and perfectly fulfilled. Hence it being called "Heaven" and "Paradise," etc.
We were all meant for Heaven. That is the plan and intent God has for us, and that is His wish. But God wanted us to be conscious, intelligent creatures who could think, rationalize, and act in ways that are free from pure instinct and natural programming. It is in this level of consciousness and awareness that we can experience the most joy and the most love. God wanted that for us. But with that level of consciousness, comes something else: free will.
This life is our opportunity to use that free will, so that we can live as the rational, conscious, and aware beings God created us as. We can ultimately choose the path God intends for us (Heaven), or we can choose against it (Hell). But that awareness and intelligence that we have as humans and that we continue to develop in this life will allow us to experience the fullness of Heaven, our intended "destination."
^Yes.

No. It is eternal separation from God directly resulting from our free choice to reject him. God does not put us there, our actions put us there.
^Yes.
"Of course, everyone will claim they respect someone who tries to speak the truth, but in reality, this is a rare quality. Most respect those who speak truths they agree with, and their respect for the speaking only extends as far as their realm of personal agreement. It is less common, almost to the point of becoming a saintly virtue, that someone truly respects and loves the truth seeker, even when their conclusions differ wildly."
