I'm fine with a doctor telling patients they are praying for them and that they are Christians, for me that's info I want to know. If they tell someone to pray for themselves they should at least know if that patient is a believer or not. If it happens again with a doctor just politely tell her/him you do not believe and go on with your life from there. I'm sure the doctor's intentions were good but a little presumptive.

It wasn't worth my trouble -- usually, I just let such comments "pass". If I think that people (such as a recent co-worker) are getting a little too pushy, I tell them that I "very much appreciate" what they are saying but that "I don't believe in that..."
