In practice..literally, it is private. The governments interaction in single payer systems is that of a collective bargainer. This is exactly how private insurance works..single payer just increases the size of the pool. The practitioners are still free..of their own accord..to practice medicine..or not..for profit.
The only way that a single payer system would be non-capitalist..by definition or by effect, is if private practitioners were not used, or it was illegal to practice medicine outside of the umbrella of a non-profit government employment contract. Our government (and every government) already negotiates and contracts a metric shitload of things....that doesn't make any of those things "non-capitalist" in either sense.
