I know that when Brad Dourif was being Pope Ratzinger Grima Wormtongue in LOTR, he stayed pretty much in character throughout, which meant retaining his Grima accent. Apparently, when others such as Sir Ian McKellen heard him speaking in his actual, native accent, they thought he was just playing about with a cod-American accent, since they just assumed he was English, never having heard him speak any other way until then.
