In the context of christianity and european culture (which would be the context that marriage has taken largely) there was little to no religious interference (the church kept records - that was about it) and not much in the way of formal ceremony except in the case of royalty (where such a rigid system would be useful to preserve the line). It could be sealed with a sentence, no fanfare, no ring. This continued to the mid 1500's (iirc).
Comparatively, other earlier (non-christian) european cultures had much more elaborate and rigid systems - though this also is not uniform.
