That disease will stop manifesting in a certain way so science can say that that disease has been eradicated but unfortunately the cause that provoke that disease is still there so will have to find a different way to manifest itself or creating a new disease or affecting the body or mind in different way.

I live in the UK and there hasn't been one reported case of polio or the Black Plague in years. I wonder if that has anything to do with doctors finding cures for them.
