Someone whose metabolism is built differently will not experience the same shit as you or your brother. I'm sorry to say that without constant exercise, it is extremely easy for me to pack on weight. The same is true of my boyfriend, who maintains his weight but does not lose any despite his constant activity at work. If you are a naturally thin person, you are not going to gain weight as fast as someone who doesn't share your metabolic rate, and a slower metabolism is going to take longer to lose weight without extra work.
Health is not a one-size fits all, and no country seems to have similar cultural attitudes about weight loss and health either.
