Defense spending would increase. Domestic programs would be reduced to the lowest levels since modern government accounting. And Medicare would be converted into a “premium support” system, where people 65 and older could purchase private insurance with federal subsidies instead of government-paid health care.

So they'd get their health insurance paid by tax dollars in order to avoid having them in a program where their health insurance would be paid with tax dollars? Or am I missing something?

It means the government would pitch in a few pennies on the dollar for the cost of any insurance they might buy.
