Yeah, I can think of only two off the top of my head. Going and shooting a bunch of people doesn't mean you were out of your mind at all. A person can do that with complete clarity of thought. Some people simply don't share the same morality as the rest of us.

The DSM entry for "spree killer*" used to specifically exempt military. Don't know if it still does, and if it has changed I  hope it's not retroactive. 

*Spree Killer: Someone who murders three or more people without a cooling off period. 
Serial Killer: Someone who murders three or more people with a cooling off period between kills.

Back in the late 70s early 80s I had two toy cap guns that were full scale replicas of real guns. I had 38 and a 9mm. After a few police mistaking them for real, the company was sued and ended up stop making them. Some BB pellet guns look just as real too.

I bought a BB gun back in 2010 that looks very dangerous at a quick glance. I try not to wave it around when my neighbor the cop is over for BBQs.
