So I had pretty much the exact same problem when I was in high school. I did well on tests and didn't do my homework. I was a B student in high school, but an A student in college where homework is normally not something that's graded. I don't really know what to do about doing homework, except to tell you not to worry about it so much. High School isn't actually important for anything.

Do your home work at school, before you go home. Don't you have free time/study halls during school? Do you have a study group of friends? If not, develop one if you can. Makes getting the work done easier. The others will help keep you on the task.
I know I'm an old fart now and occasionally memory fails but I know I hardly every took work/books home. Did it at school.
Being told you're delusional does not necessarily mean you're mental.

And that's a good thing. I wouldn't beat yourself up over it. Asking a teacher to excuse you from homework will not work. If they excuse you from homework, then every other person that studies their ass off and does the homework and gets an A gets screwed. Not going to happen. Creating accountability, like I said before, is the way to go. Ask your teacher to email or write down all your homework assignments on a sheet to be signed off by your parents. They then know what homework assignments you have, and assume some responsibility in making sure you get them done. No reward system since that doesn't work for you. Just accountability.

Sooner or later, Heat . . . 2 years? 5 years? -- You're going to have to make your own money. You're going to have to put a roof over your head. College or some sort of technical studies probably will determine just how much money you will make.
Now, it's possible not to study at all after high school, and make a decent wage. I have heard that assembly-line workers make good money. How they handle the boredom, I don't know, but they manage. Trash men and the guys who dig ditches, or climb poles to fix electrical lines make good money. So if you like being outside all the time, doing physical labor, that might be for you. Or you could study a trade. Mechanics make a decent living. Construction workers too - welders, plumbers, and carpenters. But if you want to go to college, you need decent grades. If you want to get into a good school for medicine or law, you need perfect grades. I second the ideas of parental help and peer study groups. Want to design video games? Work in TV, movies or media? Find great new uses for solar power? Perfect the self-driving flying car? Walk the surface of Mars? You're bright. It's all within your grasp. But you'll need decent grades and study skills to get there.

I use to have the same problem, about the best thing I can tell you is you HAVE TO DO ALL the little stuff. No matter how smart you are or how much you know no one will pay you much unless you have that piece of paper that says you did ALL THE LITTLE STUFF. Just do it.
I thought school was a joke and didn't want to go past high school, I figured I would work in one of the six family's businesses and there were 75 vehicles, 4 houses, and 35 aircraft. I could ace tests, read Hustler between the pages of my text book in class, and had to pay off teachers to get (A's+F's=D-'s) a C-. Right after I turned 18 my whole family basically were killed and the attorneys and thugs took everything and I was homeless, broke and had to start from zero! I had to do major hard time labor for decades and it wasn't easy and constant frustration just trying to get somewhere. The only brooks and advice were from the cops and judges handing me the joker card and rolling the stone over me on the Sisyphus hill. I recall having to go search for coke bottles after work to cash in for a pack of cigarettes just so I could momentarily take my mind off of how hungry I was. I recall never getting a full nights sleep cause there was always a cop beating on the car window in the middle of the night and make me move to another spot.
