This is a self-correcting issue. As global warming continues apace, the oceans will get larger and larger. As long as the number of ships stays roughly the same, the oceans will become less crowded and collisions will decrease. You're welcome.

How hard is to have someone looking towards the sharp end of the ship to look for things in the way?
Might be good to use radar as well

There's a theory that the ship's electronics were hacked. If that's the case then we have a really big problem.

Depending on the weather, there would be lookouts out on deck, or at least on the bridge. Sonar and radar may or may not have been in use, but I would have expected them to be. Kind of hard to miss a tanker.

The course of the ship and surrounding traffic is observed from CIC, Combat Information Center, and this is used at training for The Day. How billions of dollars of radar and computers (counting development) can miss something like a tank is a hard question to answer.

Depends on where steering was lost, of course. I helped provide "alternative hydraulic power" a few times when the pumps when FUBAR. River Platte estuary is a fun place for that, btw.
