Talk about living in a dream world.... Simplest answer to that is it doesn't work that way and you know it doesn't.
BTW, I didn't mention it before, but the biggest hospital in my area actually owns 5 hospitals and numerous clinics in the metro area, including some smaller towns. There is very little competition.
My mother lives in a more rural area about 3 hours away and she is of course advanced in age. She and my father often have to wait several weeks for appointments to see a specialist. This is due to the fact that there just aren't enough of them around. Some of them are based in a large city and have satellite offices in a radius of up to 100 miles.
County Dept of Health and State Dept of Health works that way. Then there are Federal agencies that can step in for reviews, audits and accreditation. Hospitals are required to have acceptable emergency plans.
I grew up in a rural area, I know the score. My wife has to wait months to see her eye specialist, unless it's an emergency, and we're large city adjacent.

I have two family members who work in the health care sector. My sister is head of nurse training at a small town hospital (she started as a nurse) and one of my daughters is a nurse. When they tell me stories about how their organizations are run it sounds like a nightmare.
Why is it so?

I guessit depends on where you are.
A few weeks ago I caught a metal fragment in my cornea while running an angle grinder. ( yes I was wearing wrap around safety glasses)
I went to our local hospital.
Local emergency room - on Saturday night - 1 other guy - on his way out.
I saw Dr Zzmbazzi in under 15 minutes (real name. Real good dude - I have seen him a couple times and highly reccommend him).
He had to go back to his office and retrieve his wife' s present she got him for xmas - a high quality pair of magnifying illuminated goggles - he was excited to finally get to use them. I understood completely. A trial run with a new tool is not to be underrated.
He used the goggles and a steel pick and removed the steel splinter from my eye.
I was out the hospital door 45 minutes from my arrival.
