I'm reminded of an article I read about why dictator's bridges collapse. Engineering "leadership" done by fiat is always going to fail. I remember an IG coming through one place I used to work at, and the number of millions said employer got shaved for was impressive, to say the least. All that would have had to happen was not let one little man (in the figurative sense) ramrod the design through, all the way to a very expensive equipment failure.
