Well, the physical part is pretty obvious - if your vision is poor and can’t be corrected to a certain minimum, you can’t get a license (here, at least).
As far as mentally fit goes, you need to be able to understand traffic rules and pass a test that shows you do.
I admit, though, that the psychological bit is kind of a poser. I’ve never heard of someone being denied a license because they’re depressed or bipolar or something similar (although I imagine you could lose your license if those conditions cause issues with your driving, but even that’s kind of a stretch).

Depends on their medication. IDK what OLB is going on about. We already live in that country. OFC..there are always nutballs who claim to be sovereign citizens freely traveling the country in their wagons - ipso facto they don't have to follow traffic law or licensing or registration or taxes.
