Act like you're hanging out with a friend. Be yourself, be honest, but don't feel like you have to share your deepest, darkest secrets on that first date (that can come off as creepy, but if you're connecting with your date, maybe not).
Wear something that suits whatever activity you are planning on doing.
As for what to do... I say do something that you enjoy doing, or are interested in trying out. Sharing a meal is a typical first date activity. But really, anything will do (going to see live music, a museum, wandering around a street festival, etc.). My only advice in the "what to do" arena, is make sure whatever you do, you will be able to talk to your date (ie. going to the movies isn't such a great first date, in my opinion).
