Why were they ignored?
Well we have a system that is subject to human error. You can't expect a perfect system when people are in charge. This stuff will always happen when judges decide sentencing. If certain crimes always carried certain sentences then this wouldn't happen. You can't have it both ways and then complain when it fails.
