Like others, I don't necessarily feel obliged to say anything in response to a sneeze, any more than I do to any other bodily expulsion. If I do say anything, it's most likely going to be dependent on context - such as "Can you please not sneeze on me" or "I don't think I want that pizza now, thanks".
In any case, responding with a "bless you" is merely a linguistic hangover and carries just about as much meaning as saying "good morning":

I just say bless you. I don't feel the need to get hung up on basic etiquette just because I don't believe in something.
