It has to do with early development. Initially, the embryo follows a female developmental path (it doesn't express testosterone in the first two months of development), and by then it's too late (we're stuck with mammary papilla).

Tough one ... because it's not well settled (difficult to test). Asexual reproduction arose first (single celled organisms basically just dividing), and early in the evolutionary process, either a mutations arose (creating parasitic genetic elements) or through an endosymbiotic process, bacterial proteins that were able to induce meiosis in eukaryotic cells (there's also an RNA theory of sexual reproduction).
