I need to change the code slightly so it doesn't repeat names, but I think it's helpful. I might limit it to the names used in the last 3 months though.

It would be less obtrusive if it only listed the name in parenthesis or brackets or something. Do we really need that long line of text? Even just "Formerly known as:" would be an improvement.
