I thought there was one human and one toddler?
I am the Infantry. I am my country’s strength in war, her deterrent in peace. I am the heart of the fight… wherever, whenever. I carry America’s faith and honor against her enemies. I am the Queen of Battle. I am what my country expects me to be, the best trained Soldier in the world. In the race for victory, I am swift, determined, and courageous, armed with a fierce will to win. Never will I fail my country’s trust. Always I fight on…through the foe, to the objective, to triumph overall. If necessary, I will fight to my death. By my steadfast courage, I have won more than 200 years of freedom. I yield not to weakness, to hunger, to cowardice, to fatigue, to superior odds, For I am mentally tough, physically strong, and morally straight. I forsake not, my country, my mission, my comrades, my sacred duty. I am relentless. I am always there, now and forever. I AM THE INFANTRY! FOLLOW ME!

Personally I think some of the difference lies in the way in which the people are going to die. In the trolley example, if the 5 wretched mortals die, it will be because a trolley ran over them. If the 1 wretched mortal dies, it will also be by a trolley running over him. In the organs example, the 5 people will die of organ failure, because for whatever reason they are already unhealthy and have not been able to become healthy in time to live. But the 1 person, if he/she were to die, would be a healthy person, (assuming that whatever minor problem he/she's come to the hospital for is not going to kill him/her), who died because she was deliberately made unhealthy.
That, personally, is why I find the consequences of saving 5 people to be quite different in the two situations. So yep, inconsistent.
PS: I really want to know other people's views but it's a long thread and I don't have time. I hate having to study for exams ugh.

Re the OP: Not me, I'm a wing-it kind of guy. Let tomorrow take care of itself. Get in the habit of making good choices everyday and then it won't be necessary to carry around a decision tree to predetermine your response in every eventuality. I refuse to believe I'm smarter the day I pick the best moral decision tree than I will be in the future. In fact, if I keep up the habit of choosing well every day, I might just get better at it.

I'd just like to remind you all that a trolley running over five people instead of one would get far more hits on youtube.

Yes, but would it get FIVE times more? Let's think of the views per casualty for maximum efficiency, please.

The only way to be sure is to record the death of the five people, then switch the points to the first person and wait for the next trolley to come along.
