Social stigma. Not unlike the closet gay: the closet BDSM practitioner suffers little in the way of social 'hardships'.
Once one's status is known to members of a society, regardless of what said status constitutes, there is the possibility of said (believed) status affecting one's interactions with knowing/suspecting members of said society... which is often coloured by the prevailing beliefs of the (believed) majority.
Tell your parents (or an otherwise applicable figure of authority and/or judgement in your life) that you like to put a collar on your man and that he enjoys nipple clamps... and tell me you (and your man) don't suffer 'hardships' resulting of such.
Tell your employer, and see if you're not fired right quick... bring it up on a job interview, and your chances to get in have become virtually nonexistent.
That 'general sexual rights' of persons are not commonly fought for under title as such does not mean that they shall forever remain 'in the dark'.

It can also be a compulsion. When normalized violent trauma disappears on a sudden, humans sometimes seek it out... it can feel 'right', even when it hurts. It can be hard to walk away from what one is used to experiencing.
Also, if one royally fucks up, and they don't know if they can adequately 'punish themselves', said individual may request the application of pain.

There are many more ways. Personally... I'll take a little pain to further a worthwhile end. Others might well do it with the intent of shielding another, which is not always threatened by another, but which the one granting themselves in <capacity> might well be cognizant of.
Obviously, this goes a lot deeper than physical violence, and is a manipulation tool constantly under use (take an annoying task or whatever for <reason(s)>).
