That seems to be exactly what he's saying in that verse. Not that it's a strict command, but a suggestion that we all know some followers would take seriously enough to do it. How many times do you think millstones get accidentally hung around people's necks?
Maybe he just wanted his followers to make it look like an accident. It would be a shame if a nonbeliever found a millstone hung around his neck, and fell into the sea with it.
