The cutoff for "small" is usually defined as the average value minus 2-sigma (two times the standard deviation).... I don't know what the distribution of penis size is, but it is known that a large enough sample will lead to a "normal" distribution, so the 2-sigma has a definite value. Again, I don't know what it is.

Concerning un-circumcised length... if it adds length it's only because the skin was cut poorly the guy can't enjoy a full erection.
Also, when erect, the foreskin gets retracted, so it never counts for length.
