I think it is irrelevent whether they were born that way, whether it was purely environmental factors, or whether it's a combination of both.
The bottom line is that for whatever reason, a person IS "that way", and not by choice. A transgender person feels like they are not the sex that they actually are. They are not making it up, or choosing it. It is a legitimate unfortunate condition that they are faced with. I will call a grown adult whatver they want me to call them, even if it doesn't line up with their sex. It's just a matter of respect.

Most respect those who speak truths they agree with, and their respect for the speaking only extends as far as their realm of personal agreement. It is less common, almost to the point of becoming a saintly virtue, that someone truly respects and loves the truth seeker, even when their conclusions differ wildly."

A disorder or illness, though I'm not a doctor to understand the differences between the 2 or which one would apply.

When the theist refers to my sexual orientation as an illness, s/he shouldn't be surprised when I refer to their religious beliefs as manifestations of delusion.
