I've never experienced it yet myself... and I'm hoping to never do so. If you've ever been on the phone with me: you know you have 48 seconds to spit out whatever it is unless you haven't talked to me in forever and are a very close friend... which you are probably not
My circle of friends actually created a protocol to avoid that situation. When you are done, you say "Is that all?". If the other person is done he says "That is all." amd immediately hangs up. There's just a mutual understanding, and it works great.
