To be totally honest, the Iron Cross symbol is ambiguous, at least in and of itself. I can remember, I think it was last year, seeing a rat rod with an Iron Cross on it. Apparently, according to a forum post I found on the subject (feel free to take this rationale with a grain of salt; coming up with a proper history the origins of folk traditions like rat rodding on such short notice isn't easy):

Tl;DR: A symbol like the Iron Cross is not quite a smoking gun (other groups that aren't hate groups still use it, even if they are shrinking), but in conjunction with all the other shit, it's clear that he does, in fact, count as a Nazi. Many alt-righters might count as mere useful idiots (Sargon of Akkad comes to mind) and not full Nazis, but Milo ain't one of them.
