"How do we distinguish our current (supposedly real) interactions and understanding of reality from a delusion (such as when under the influence of drugs) about which the one experiencing the delusion could be equally as 'certain'?"

Yes.
A person may be on drugs and that person may be hallucinating. However the point is not that, 'oh he is on drugs' forget how the brain was altered or what physical thing was used. The point is, the brain is capable of seeing, interacting in such a manner, which just is impossible to comprehend in the normal world.
Furthermore, I am not taking about a foreign substance which changes the way your brain works, DMT is already naturally produced in the brain.

If the brain is capably of being in a world where such unimaginable things can happen, why is it in the current level consciousness which it is in now? It is possible for it to produce more DMT and then suddenly the world we currently perceive is no longer.

I can't put my tongue on it... the only factor is time, which differentiates the 'two worlds'.

Imagine a timeline which stretches a billions years.

for a 1000 years humans live in the level of consciousness which I described, then for the remaining 999000 years humans live in our current level of consciousness.
The 1000 years is insignificant in the timeline, is it not? At the time for the humans  during those 1000 years that would have most definitely have felt the 'norm' for them.
But when you stretch the timeline you will not even notice those 1000 years, however it did exist.
Now if we look at that in our say 70 years of life we live. For 70 years we live our 'normal' level of consciousness, however for an hour out of those 70 years a person lives a completely new level of consciousness experiencing a completely new 'world' (by increasing level of DMC in the brain). The time is so short in the 70 year life, it can be passed as insignifcant, however it did exist. if that 70 years were to be expaned to a 70 billion years that 1 hour would equate to nearly a 1000 years. Imagine living in this mind frame for a 1000 years. This is just a matter of time.
