Looks like a transgender man who still has a lady garden and has got pregnant. Wouldn't be the first time, it's happened before

For starters, when a woman is that pregnant, there is a dark line that extends from the belly button south to the nether regions. That's missing in the photo. Second, small hands yes, but rather a bit manly looking. Third, the arms don't look feminine enough.

I'd say it's a beer baby. Or better yet, I think Yeauxleaux gives the best answer.

I unfriended a woman last year for insisting that a picture of my dick that I sent her was actually a clitoris, so I totally understand her reaction.
It was very cold when I took the pic.
