As a nurse I can't really "answer" that question.  Instead, I would have to state, "that's a question for your parents".

If by the term "about to die", you mean within the next minute or so, I don't think it really matters at that point. Besides, if the child wasn't mine, it wouldn't be my business to say anything. I'd be curious to know why the parents weren't there.

If the child was mine, they already don't believe in a sky fairy, so it's a moot point.

If you're just going to lie anyways I would tell them an elaborate story about going to a place where they can ride unicorns, cats and dogs live in harmony and we can eat ice cream everyday.  Might as well spice it up kid style.

I'd lie to him/her and say "Yes, you're going to Heaven." Because theists have made that acceptable.

If they're a believer I'd just let them believe. If they are that young (most likely lacking critical thinking skills) and dying, whats the point in causing additional trauma? 

If they're older (capable of critical thought) I'd just turn it back on them and ask "What do you believe?" and take it from there. 

It's not my goal to convert people to atheism. They can believe whatever they want to no matter how silly it seems to me. I wish more religious took this position regarding atheism but alas, most seem to be compelled to be dicks and assholes insisting that I validate their beliefs while they invalidate mine (or the lack there of).
Being told you're delusional does not necessarily mean you're mental.

I'd probably tell them I've never seen anyone more likely to qualify in my life.
