If they genuinely think of themselves as a lesbian, then they'd have to be a transwoman. Hehe... there was an instance where I tried to explain my condition immediately after math class as specifically 'a lesbian in a man's body', but the boys of the class all claimed to relate... at which point I realized that they totally weren't thinking about their vagina being eaten out

I'm assuming you mean a male to female transsexual woman who is also attracted to other women, correct? Generally, just a lesbian, but it depends on the context. You use whatever terminology is relevant to the situation at hand. The term 'transbian' has been thrown around a bit, too. In fact, when I was really early on I met a girl who has since become a big porn star who calls herself a transbian (I'd link you to her site but, sadly, this isn't area 69).
