I've already told you....limited to your constraints I'd give him the medicine and look for a way out. In reality I'd find one, in your scenario I wouldn't...but I don't do the shitty thing just because the right thing is hard or impossible.

We're not talking about reality here. We're talking about this imagined case in particular, and nothing else. And in that particular scenario there is no other way out. You need to make a choice. I presented you with your only two options. Will you tell me which it is you would take or not?

It's fairly simple. You either pull out the medicine or the knife. Tell me again which it is, as a definitive answer, without any allusions to a third option or a way out, and we'll move on.
