Also - seeing a sleep pathologist might help.
I know what you mean about your doctor refusing to prescribe. I have chronic pain and can't get a pain Rx to save my life. Fucking DEA has all the docs scared.
If you are looking for otc help, have you tried melatonin? Since you take prescription meds, you should probably check with your doc, but I've foud it very helpful without any nasty groggy side effects or drug interaction issues.
Other than that, just all the sleep hygiene things people mention. If you can't sleep, get out of bed and read on the couch, or sit in a warm bath. Something not too stimulating. Staying in bed when you are restless and not sleeping makes it harder in the long run. Etc, etc. Sounds miserable though. I hope you find something that helps you soon.

I toss and turn between my belly and both sides whenever I feel the urge as I go to sleep and I probably do it while I'm unconscious too. If I suppress my restlessness I can't even sleep, if I allow myself to be restless I sleep like a baby.

Than pain shit can get rough but I think my pain management doc likes me better than your doctor because I do take a lot more pain meds than most people. I also could use about double the amount that I take if I wanted to actually be pain free. I feel more tired instead of less tired. It's very frustrating. I've struggled with insomnia off and on a lot but I'm now more exhausted when I actually am sleeping. I think I need to get a kickboxing bag to take out my frustrations about being so damn tired.
