If you can manage it, pay one of her bills for her - doesn't need to be anything major, but this month's credit card installment, her mobile phone charges, etc. That way, it isn't so much a 'gift' gift, but it both serves to show your appreciation and gets you off the potential hook of not doing anything at all.

I always make something for my ex's birthday, with the help of my daughter.
This year we bought a small, rectangle, concrete tray, filled it with sand and placed seashells, rocks and driftwood on top.
Then I used a barbie doll to put footprints walking through the middle.
I'm actually working on another one at the moment. I've cut a small coconut shell in half which I'll fill with hydroponic sand and put a miniature cactus inside. :-)

I do know her but with the gifts I've bought her in the past I'm running out of ideas, possibly something funny or a bit weird or outside the box.

Give her a "Happy Birthday", and spend the money you would have spent on her gift to buy a nice leather collar.

You know you like that sort of stuff anyway.

How about writing her a heartfelt letter about what she means to you, how much you love her, etc. On nice stationary or course.

Just as a side note, it's not girl-speak to say you don't want something and really do want something. Sometimes, I tell Tibs I don't want something because I genuinely can't think of something I need/want and don't have. He always manages to find something, though. My point is, maybe she just really doesn't have room in her life for more stuff, so, as people have mentioned, an experience is probably better.

What I ended up buying was a voucher for Starbucks because she mentioned going there on her break from work.
I got her a more expensive voucher for places where you can buy roast dinners because she likes English food, I think English food like that is really bland it boggles my mind that someone would choose to go to one of those places but I also offered to go with her.

On her card I told her not to worry because she still looks 19.
Shell be happy probably.
