Amongst us engineers it is called "design verification", or "engineering test". After you have designed someting, you put it to the test if it fulfills the requirements. When it fails one test the design will be marked as "fail" and be blamed.
...oh, wait....

the design then gets updated/improved....btw. Where is "human 2.0"?...and tested again. And again it gets blamed when it fails. The design then gets fired from its job and a new and better design hired....oh, wait...
