There are two passages used to say masturbation is bad in the Bible. The first is the one Robvalue mentioned about Onan spilling his seed (also used to forbid contraception).
The other is from the New Testament where Jesus says that looking at a woman lustfully is as bad as adultery, followed immediately by a passage saying it's better to cut off a hand or an eye that causes you to sin and enter heaven maimed than keep the part and go to hell... Of course, without masturbation explicitly listed as a sin, the whole "hand causing you to sin" thing is actually a circular argument; however, most people tend to think naughty thoughts when playing with themselves, so a Christian could make a straight-faced argument that the lustful thoughts are the sin, and not the self-induced orgasm.
Short answer: a lot of cultural baggage based very loosely on the Bible.

How difficult would it be to achieve a self-induced orgasm, without lustful or sexual thoughts of any kind? Possible, but extremely difficult, I would imagine...probably requires monk-like focus and concentration.

Thoughts without contact is easier than contact without thoughts. Arousal is paramount.

Without arousal it's like scrubbing sandpaper.
I'm pretty sure both are paramount.
Let's just say I've never had an orgasm without arousal but I have had one without stimulation: Wet dreams.

I can safely say I have never succeeded at it. And yes I have tried. I end up lasting for ages and eventually lose interest.
Then when I put my mind to it again, I can maintain interest and finish.
