Now before you answer lets look at two points of reality
1) We currently don't have enough doctors to cover the people who are covered
2) Medical error is the 3rd leading cause of death in this country

So to achieve what you are looking for you need to force people who would be good at being a doctor, to be a doctor while removing the doctors who are doing "substandard" work. Given the speed of medical advances this likely means that the effective number of years a person could be a doctor is around 15 before what they know is outdated and substandard. Somehow you want to take a system that already needs more doctors, get rid of the ones who aren't performing well (likely 30 - 40%), and magically expand those "good" doctors to cover all 320 million people in this country.

Look I get you want a fantasy, but don't present it like I'm this horrible person because I won't accept reality. Humans aren't clones, some doctors are going to be better than others, and some people with less knowledge and training would be better doctors than the doctors we currently have. Being poor doesn't inherently make you bad at something, and not everyone wants to spend a decade before they start applying what they are learning.

I'm still not seeing any reason to resort to substandard care Cov.  Unless a "t1" doctor is at least up to our current standards, they have no business opening people up.  If they are, I fail to see the point of a tier system in the first place.

A students impatience is probably the silliest justification for lowering our standards of care that I've ever heard, it's not even out of left field....that's space cadet shit.
